HammondCare provides palliative care education and training for both residential aged care and at-home care settings. A range of topics are included, such as palliative care considerations for people with dementia, pain management in palliative care, and grief and loss.

Palliative Care Bridge

The Palliative Care Bridge is a state-wide, palliative care online education program coordinated and delivered by the HammondCare Consortium, comprising HammondCare, Sacred Heart Health Service and Calvary Healthcare Sydney.

Palliative Care Bereavement Counselling Services

Bereavement counselling is a confidential discussion with a specially trained professional who can support you to explore the impact of your loss and identify strategies for coping as well as possible. Family and friends of someone who has died and who was known to a palliative care service (HammondCare or another service) and who lives in the Northern Sydney Local Health District or the South West Sydney Local Health District are able to utilise this service free of charge.
