Community Palliative & Supportive Care
Community Palliative & Supportive Care aims to help you stay at home during a life-limiting illness.
We aim to provide comprehensive support for the patient, their family and other carers. We offer support which embraces physical, psychological and spiritual needs.
HammondCare Greenwich Hospital provides comprehensive palliative care services including a community support and back-up service to people in their homes, an inpatient service and 24-hour telephone advice.
Where can I find this care?
The Community Palliative & Supportive Care Service was initiated in 1989 for people with life-limiting illnesses. It covers the municipal areas of North Sydney, Mosman, Lane Cove, Willoughby and Ryde.
A multidisciplinary team works in consultation with your local doctor, Northern Sydney Home Nursing Service and other community support agencies to enable you to remain at home.
Admission to the Palliative & Supportive Care in-patient unit at HammondCare Greenwich Hospital is also available. This can be valuable in providing respite for your family, or specialised symptom management for you.
What help is provided?
The Community Palliative & Supportive Care Service directly supports you if you are experiencing discomfort or distress, and carers who have concerns related to your illness. The service also provides consultation to local medical practitioners and other health professionals with regard to palliative care issues.
What other assistance is available?
Advice is available regarding equipment which could improve your independence and comfort, as well as making it easier for your carers to provide assistance safely. A range of equipment is available on a loan basis, including:
- Shower chairs
- Walking aids
- Pressure care aids
We have information available on a range of other services which provide assistance to people in their homes, and we will help you access these as necessary. - Emotional support
- Help to support children
- Resources and in-home respite for families and carers
We are happy to arrange a visit to our inpatient unit for those who are interested.
The Palliative Care Home Support Program
The Palliative Care Home Support Program provides end of life care for patients who wish to remain at home. A package of up to 48 hours of specialised supportive palliative home-based care, day or night, is provided by specially trained community workers who work as part of the existing multidisciplinary Specialist Palliative Care teams.
Care support delivered under the package is individually agreed between the patient or carer and the Specialist Palliative Care team and may include assistance with activities of daily living, personal and domestic care support, shopping and household assistance - providing carers with vital support in end of life care. In consultation with the Specialist Palliative Care team and the patient’s GP, it may also include basic nursing care. Care delivery may be consecutive hours or may be spread over days or weeks.
